Job Summary

Provide nursing support to assigned individuals.

Job Description


  1. Monitor the medical, dental and medication needs of the individuals assigned to ensure that health services outline in the service plan reflect the individualís needs.

  2. Meet regularly and as needed with nursing supervisor and other nursing staff to discuss programs or other issues relative to the care of the individuals.

  3. Meet regularly and as needed with assigned residential support professionals to discuss the medications and medical issues involving individuals.

  4. Consult, when necessary, with other service providers on behalf of individuals.

  5. Evaluate and monitor the assigned individualís capability of medication self-administration and self-directed status upon admission and at least semiannually.

  6. Review and perform medication regimens at least semi-annuals for assigned individuals.

  7. Provide, or arrange for, direct nursing services for the individuals as needed.

  8. Accompany or arrange for medical/dental appointments, as appropriate.

  9. Provide on call availability for medical needs for individuals.

  10. Provide medication certification training including yearly recertification to staff per regulations.

  11. Provide or assist with training as needed for Developmental Disabilities Services staff, volunteers and/or individuals in health-related areas, general medical overview, blood borne pathogen and other infection control and medication administration.

  12. Assure proper implementation of medical policies and procedures that apply to Developmental Disabilities Services programs as assigned.

  13. Attend mandatory training.

  14. Other duties as assigned by supervisor.


  1. Registered Nurse, State of New York.

  2. Minimum of one-year RN experience preferred.

  3. Previous experience working with individuals with developmental disabilities also preferred.


  1. Adheres to all applicable federal and state laws and regulations including, but not limited to, those governing confidentiality, privacy, program, billing and documentation standards. All duties must be performed in accordance with CCDRís corporate compliance and ethics program.

  2. Possess excellent verbal and written communications skills.

  3. Previous experience working with diverse populations, including low to moderate-income families.

  4. Ability to multi-task and prioritize duties.

  5. Ability to travel in all weather conditions throughout the counties served.

  6. Willingness to foster agency, department and program wide cooperation and team work through use of positive/constructive communication techniques.

  7. Ability to maintain absolute confidentiality in regard to all records reviewed including individualís records, employee records and billing records.

  8. Proficiency and experience with PCís and Microsoft applications.

  9. Ability to analyze and interpret data and to handle problem resolution.

  10. Possession of a valid NYS Driverís license and a driverís record considered acceptable by agency and insurance carrier.

  11. Continuous use of a reliable, registered and insured vehicle.

  12. Demonstrate commitment to Agency Mission Statement.

Ability to meet the following physical requirements with or without reasonable accommodation:

  1. Use hands to manipulate, handle, feel, and control items or equipment, including motor vehicle;

  2. Stand, sit, walk, bend, stoop, kneel, and reach;

  3. Climb up or down stairs;

  4. Able to reach above or below shoulders;

  5. Occasionally lift or move objects weighing up to 10 lbs.

  6. Sitting at a desk or in a vehicle for long periods of time to perform certain job functions;

  7. Be able to read write and interpret written documents.
