Job Summary

The Care Team Coordinator (CTC) is responsible for providing clerical and administrative support necessary to assure delivery of clinical care for the patient and staff schedules and planning for a given interdisciplinary team. Provides in house planning and scheduling support for home visiting staff to ensure patient visits are covered in accordance with agency standards and plans of care. Coordinates and provides excellent customer service for patients and Professional Home Visiting (PHV) staff.

Job Description



Clients, families, caregivers, physicians and office staff, community, vendors, contract and regulatory community agencies, Signature Care departments and staff, UR Medicine Home Infusion Pharmacy staff


  1. Coordinate’s chemotherapies disconnect times from cancer infusion centers with Home Infusion, URMHC CHHA, and other home care agencies. Updates Home Infusion calendar with times and assigns visiting nurse.

  2. Proactively schedules and plans unassigned visits on the Home Infusion calendar.

  3. Prints delivery tickets for routine visits for Home Infusion patients.

  4. Communicates cancelled chemo visits with nursing agencies and providers (for Home Infusion patients via Care Tend note) and deletes delivery tickets for cancelled disconnect visits.

  5. Assists Intake Specialist with referral process.

  6. Maintains nurse workday calendar, including weekends, on-call, and time off.

  7. Provides clerical support to the team. Assists field nurses with paperwork, printing, and scanning as needed. Compiles Start of Care folders for field nurses.

  8. Provides internal/external customer support, resolving all team based non-clinical customer issues as able.

  9. Demonstrates ICARE values and behaviors. Promotes positive, supportive, respectful communication to all patients, families, referral sources, physicians, and URMHC staff.

  10. Maintains patient and staff privacy and confidentiality at all times according to established policy and procedures.


  1. High School Diploma or Equivalent required. Associates Degree preferred.

  2. Minimum 2 years’ experience in community health agency or medical setting preferred

  3. Knowledge of Medical terminology

  4. Excellent interpersonal skills

  5. Effective oral and written communication skills

  6. Highly organized with strong administrative skills

  7. Works with team to assure maximal efficiency of resources

  8. Takes initiative in problem solving issues.
